Frequently Asked Questions
Support Coordination is an NDIS-funded support that helps participants understand and implement their NDIS plan. A support coordinator works with you to connect you with appropriate service providers, establish support agreements, and ensure your plan is delivering real outcomes. It is available to participants whose NDIS plans include funding for Support Coordination under the Capacity Building budget.
Support Coordination assists most participants with navigating and implementing their NDIS plan. Specialist Support Coordination is designed for participants with more complex needs — such as those involving multiple providers, high levels of risk, or involvement with other systems like health, justice, or child protection. Specialist coordinators have advanced qualifications and experience in managing these higher-complexity situations.
Check your NDIS plan document under the Capacity Building — Support Coordination category. If funding is included, you are eligible to engage a Support Coordinator. If you’re unsure, contact Grace Care on 1300 069 338 and our team can help you review your plan.
Yes. You are free to change your support coordinator at any time without needing a plan review. Simply contact Grace Care and we will guide you through the transition process to ensure there is no disruption to your supports.
Absolutely not. Your support coordinator is there to inform, connect, and advocate — not to make decisions on your behalf. The frequency of contact depends on your needs and preferences. Some participants prefer regular weekly or fortnightly check-ins, while others may require contact only when specific needs arise.
