There are many reasons families and participants reach out to Grace Care. Here are the most common ways we can help:
- New Participant Enquiry If you or a family member is looking for NDIS support for the first time, contact us for a free consultation. We’ll review your NDIS plan, discuss your goals, and explain exactly how Grace Care can help you get the most from your funding.
- Existing Participant Support If you’re already supported by Grace Care and need to speak with your support worker, coordinator, or our admin team, call us directly on 1300 069 338 or email info@grace-care.com.au.
- Support Coordination Enquiry If you need help understanding or implementing your NDIS plan, our Support Coordinators are ready to assist. Contact us to arrange an initial consultation and plan review.

Making a Referral to Grace Care
Grace Care welcomes referrals from healthcare professionals, allied health providers, hospital discharge teams, community workers, and support coordinators across Greater Sydney.
Our referral process is streamlined and responsive, prioritising prompt onboarding for referred participants to minimise gaps in support.
